MLIS Student Program Forms

Add/Drop a Course

Please note: If you are dropping your only course for the semester or summer, you will need to fill out a Leave of Absence form even during the Summer term.

See Student Accounts for the Tuition Refund Schedule.

Student should submit this form to drop a class after the online registration has closed. Students may need to obtain an instructor's signature, using the add/drop form, to add a course after this date. If a student is dropping all of their courses, then they need to fill out a Leave of Absence/Withdrawal form

Computer Competency

Students Admitted Prior to Fall 2004

Students admitted prior to fall 2004 must demonstrate computer competencies as outlined in the competency project form.

Course Planning Worksheet

The worksheet will help guide students in planning out their courses to complete the MLIS program.

Incomplete

Along with other forms, the petition for an incomplete is online and available through the registrar (scroll down to the "Office of the Registrar Forms" heading and click the appropriate link). You will be required to log in using your email username and password. Once completed, you select the faculty member to whom you are petitioning for an incomplete. That faculty person receives an email alerting him/her to the fact that the petition has been made and requires approval or denial. The student is then notified of the instructor's decision.

Independent Study

Students wishing to pursue a topic that is related to library and information science may register for independent study (LIS 801 Special Studies in Librarianship). Students work closely with a faculty member whose expertise and interests lie within the proposed topic area.

Independent studies are designed for students who have completed eight courses of their program and have a grade point average of 3.3 or better or with permission of the program director. No student may complete more than two independent studies.

Students must have sufficient background to work independently and must develop a proposal for independent study that meets the approval of the faculty supervisor and the program director. The independent study topic should be of sufficient academic and scholarly substance and merit to be worthy of at least three credits, and it should result in a product that will document both the substance and merit of the student's performance in the independent study. An independent study is not a substitute for taking a class.

Intent to Graduate

To prepare for graduation, print off the Dominican University Intent to Graduate form, fill it out, and mail it to: Dominican University-GSLIS, Office of the Registrar, 7900 West Division Street, River Forest, IL 60305. In general, the form is due two months prior to graduation, but check the form itself for exact term deadlines. You need to also send an email to IMDept@stkate.edu with your name, ID number and that you intend on graduating in month and year. Then check it off on the graduation checklist.

Students completing the MLIS degree through St. Catherine University should immediately go to the St. Kate's graduation checklist for directions. Students should begin considering the graduation process at least one semester ahead of the semester they would like to graduate. St. Catherine University deadlines for the Declaration of Intent to Graduate form are May 15 for fall Semester graduation and November 30 for winter Semester graduation. (Fall 2009 deadline has been extended to September 15, 2009.)

Leave of Absence/Permanent Withdrawal

Please note this Leave of Absence/Permanent form is only for students in the Masters of Library and Information Science program. A Permanent Withdrawal is when you are leaving the MLIS program and do not plan on returning. A Leave of Absence is when you temporarily leave the program and plan to return.

If you need to take a semester off or withdraw from the program completely, please print the leave of absence/withdrawal form, fill it out, and then submit it to the MLIS Offices, room 043 in the Coeur de Catherine or mail to: MLIS #4125, St. Catherine University, 2004 Randolph Avenue, St. Paul, MN 55105.

See Student Accounts for the Tuition Refund Schedule.

If you are dropping all of your courses for the semester or summer, you must fill out a Leave of Absence form. It is the student's responsibilty to turn in this form.

LIS 7993 Practicum

Before considering a practicum, download the practicum form and read the guidelines to understand what is required. For instance, a student can take LIS 7993 only after the core curriculum is completed. When you are ready to complete a practicum, make an appointment with Mary Wagner, Program Director, to discuss it. Bring the downloaded and completed form with you.

The prerequisites for LIS 7993 are ten LIS courses including the four core courses and a grade point average of 3.3 or higher.

Permission to Take Excess Credits

Three courses per semester are a full credit load. You must obtain written permission to take more than the full credit load from the program director, Mary Wagner. Students are not generally advised to take excess credits and should speak to their advisors before doing so.

Petition to Transfer Credit

This form is included in application materials. Should it be misplaced, however, you can download and print the MLIS Petition to Transfer Credit from here. More information here.
